June 13, 2013 - CDPH announced today the issuance of three separate Class “AA” citations, the most severe penalty under state law, to California skilled nursing facilities, along with fines totaling $200,000.

June 6, 2013- CDPH issued 10 penalties today to Calif. hospitals along with fines totaling $625,000 after investigations found the facilities’ noncompliance with licensing requirements caused, or was likely to cause, serious injury or death to patients.

May 16, 2013- Horizon West-Lincoln, a skilled nursing facility in the city of Lincoln, has received a Class “AA” citation, the most severe penalty under state law, and a $100,000 fine from the State of California.
