RFA Clarifications, Corrections, and Modifications

Statewide

  1. The due date for applicant questions on this RFA has been extended to January 24, 2012.  CDPH will post responses on a flow basis until January 26, 2012.  Please refer to Section 4, Provision D, of the RFA for instructions on submitting questions.

  2. The instructions for completing Forms 6A-1, 2, and 3, Work Plan Narratives, in Section 8, Forms Instructions, have been amended.  Provision 2.b., in its entirety, on pages 66 and 67, has been eliminated.  In addition, Provision 2.d., on page 67, has been eliminated.
  3. Section 2C, Page 30, 14.0.2:  Network Administration (NA) and Network Engineering (NE) Services states "Air Force directory support," it should state "Active Directory support."

  4. Section 2D, Page 40.  Item 4.4 is missing (4.3 goes to 4.5).  This is a clerical error.  4.5 should read 4.4.

  5. Section 4D, Page 45 has a reference to December 22, 2012, which should read December 22, 2011 as the correct date.

  6. The direct and primary recipient must perform a substantial role in carrying out project objectives and not merely serve as a conduit for an award to another party.



Regional

  1. The due date for applicant questions on this RFA has been extended to February 28. 2012.  CDPH will post responses on a flow basis until March 1, 2012. Please refer to Section 4, Provision D, of the RFA for instructions on submitting question.

  2. Page 39, Instructions for completing Form 6A, Work Plan Narrative, in Section 8, Forms Instructions, have been amended.  Provision 1.b. in its entirety has been eliminated. In addition, Provision 1.d. has been eliminated.

  3. Page 8, Introduction, paragraph 4, "After collection and processing at the regional level, the data flows to the central registry’s Eureka data management application."
  4. This statement is inaccurate as stated.  All cancer cases collected from all reporting sources such as hospitals, physician offices, pathology laboratories and related source document data will be uploaded directly into Eureka (or manually entered into Eureka), and be processed in Eureka by Regional Registry staff.

  5. Page 9, Section C, bullet 6 states: "Provide necessary resources and epidemiologic expertise to use regional and data to:
  6. "This supposed to state: "Provide necessary resources and epidemiologic expertise to use regional (delete and) data to"

  7. Page 11, Section 4 states: "The Applicant shall support statewide data management activities by participating in Eureka testing and supporting the recruitment and implementation of electronic reporting from regional pathology and other facilities."
  8. This statement should actually state: "and implementation of electronic reporting from regional pathology laboratories and other facilities."

  9. Page 30. "Raters will consider the following when scoring the Applicant's Approach: Do the proposed regional registry activities adequately address the mission and priorities of the CCR?"

    The mission of the CCR is given at: http://www.cdph.ca.gov/programs/ccr/Pages/AboutUs.aspx, but the CCR priorities are not specified. A correction has been to the RFA to only address the mission of CCR, which is available at the link provided.

  10. Page 33, Agency Capability, 10th bullet. "Has the applicant provided sufficient justification of any data collection costs that may exceed the statewide average (such as cost of living, rural area requiring more extensive)?"
  11. This bullet is being eliminated from the Evaluation Criteria that raters will consider when scoring the applicant’s cost.
