Continuing Education Requirements 

Clinical Laboratory Science, Cytotechnologist, and Director Licenses

Everyone who holds a clinical laboratory scientist personnel license type (including limited scientist, cytotechnologist, and director) must obtain 24 hours of continuing education approved by clinical laboratory science continuing education accrediting agencies or from an accredited academic institution during every two-year renewal cycle. 

Phlebotomy Certificates

Certified Phlebotomy Technicians must obtain a total of 6 contact hours of continuing education approved by clinical laboratory science continuing education accrediting agencies or from an accredited academic institute during every two-year renewal cycle.

You must retain for a minimum of four years all continuing education documents received from providers approved under the California Code of Regulations, Title 17, Section 1038.4.  Do not forward these documents to the Department unless you are instructed to do so.

How to Renew a Certificate or License

Quick Checklist:

  • Obtain the required number of continuing education credits (see requirements above).
  • Complete form LAB 177, including the Continuing Education section on page 2 of the form.  Be sure to sign and date the form.
  • Submit a check or money order for the non-refundable renewal fee made payable to the California Department of Public Health.  Be sure to write your license or certificate number on the check.
  • Mail the form and payment to the following address:

  • Keep a copy for yourself and make a note of the date and where it was sent.

 

Renewal Information: 

To renew a certificate or license, the certificate or license holder must complete and return form LAB 177 along with the appropriate fee.  Renewal fees are not refundable.  View the schedule of renewal fees.

Renewal notices are sent to licensed and certified persons 120 days prior to the expiration date of the expiration of their license or certificate.

If you have misplaced your renewal form or did not receive it, the renewal form LAB 177 is available online on the LFS website at Laboratory Field Services Forms.

Be sure to complete the Continuing Education Activity Summary found on page 2 of form LAB 177.  The following information must be provided for each continuing education program that has been successfully completed: (a) date of issue of the continuing education certificate, (b) California Approved Accrediting Agency number (AA number), (c) program title, (d) course number, and (e) number of contact hours (CE hours) received.  The renewal form will be returned to you with a letter of deficiency if any information is missing, and this will delay the processing of the application.

It takes a median time of 90 days after the receipt of a complete renewal application to process the application and issue the license or certificate.  To ensure renewal before the expiration of your license or certificate, the complete renewal application package (form LAB 177, including a list of continuing education courses, and fee) should be sent at least 90 days prior to the expiration date.  An incomplete or incorrect application will delay the processing of the application.

Applications are processed in the order in which they are received.  Please note that LFS does not have a public counter and does not accept in-person requests or applications, including applications for renewal.

After an application has been approved, the approval is posted on the LFS personnel verification website.

LFS does not issue temporary licenses or certificates or verification letters.  Employers and license or certificate holders can verify renewal on the verification website.  Please note that it can take up to 10 business days after approval of the license or certificate for the information to appear on the verification website.

A screen print of your verification page can be used as proof of licensure or certification for employment purposes for 90 days after the date of approval or until you receive a paper copy of your license or certificate. The screen print must be attached to your expired license or certificate.   

 

How to Order a Duplicate License

  • Complete the duplicate license form LAB 176
  • Submit a check or money order for $17.00 made payable to the California Department of Public Health.  Make sure to write your license or certificate number on the check or money order. 
  • Mail the payment and form LAB 176 to the following address:

How to Submit an Address Change

You are required by regulations to notify LFS in writing of a change of address within 30 days after the change has occurred.  Submit a written notice to LFS by mail, email, or fax to the attention of the Personnel Licensing section.  The notice should include the following information: 

  • License or certificate number
  • Printed name
  • Old address
  • New address
  • New phone number
  • Email address
  • Signature

Your new address will be entered in the LFS data system.  You will not receive a new license or certificate.  The next time you renew your license or certificate you will receive your renewal notice and your new license or certificate at your new adddress.  

 

How to Submit a Name Change

You are required by regulations to notify LFS in writing of a change of name within 30 days after the change has occurred. You must obtain a new license or certificate showing your new name.

  • Complete the duplicate license form LAB 176.
  • Submit a written notice of the change of name to LFS.  This notice should include the following information:
    • License or certificate number
    • Printed previous name 
    • Printed current name 
    • Address
    • Phone number
    • Email address
    • Signature
  • Submit a copy of the court document or other government-issued document that shows both your previous name and your new name.
  • Submit a check or money order for the $17.00 duplicate license fee made payable to the California Department of Public Health.  Make sure to write your license or certificate number on the check or money order.
  • Mail the package, including form LAB 176, the written notice, documentation of name change, and payment, to the following address:

You will receive a new license or certificate with your new name, but the expiration date will remain the same as on your old license or certificate.  The new license or certificate with your new name must be posted at your work location.
