Guidance for Report= ing Registered Facility Changes

 

In accordance with = the California Code of Regulations (CCR), title 17, section 30115, the registr= ant shall report, in writing, to the California Department of Public Health, Radiologic Health Branch (the Department) any change in: registrant's name, address, location of the installation or receipt, sale, transfer, disposal or discontinuance of use of any reportab= le source of radiation within 30 days of the change.  The Radi= ation Machine Registration form (RH2261) is used both to register and to report requ= ired changes.  <= /p>

 

Every person who acquires a reportable source = of radiation not operating in excess of 500 kVp s= hall register the X-ray machine within 30 days of the date of acquisition. For additional information on removing an X-ray machine registration, see the = Guidance for Disposal of X-ray Machines.=

 

Special requirements for mammography machines and X-ray mach= ines operating above 500 kVp are listed and liked b= elow.

 

Every person who ac= quires X-ray machines that operate in excess of 500 kVp must notify the Department at leas= t 60 days prior to his/her possession of the machine or at least 60 days prior = to the commencement of construction or reconstruction of the room which will house the machine, whichever occurs first.  See = the guidance document Guidance for Acquiring Radiation Machines Operating in Excess of 500 kVp.  This equipment shall not be used to treat patients until written approval of provisions f= or radiation safety has been obtained by the user from the Department.

 

Mammography equipment (screening, diagnostic, and/or intervent= ional) shall not be used for human patients prior to the Department’s appro= val and until a current and valid Mammography X-ray Equipment and Facility Accreditation Certificate is posted on each mammography machi= ne. See the guidance document Mammography Fact Sheet. You must complete the Ap= plication for Mammography Machine Certification (CDPH 8623) and submit the appli= cation with all supporting documenta= tion. The Mammography X-ray Equipme= nt and Facility Accreditation Certificate is valid only for the facility owner, facility name and location fo= r which the certificate was originally issued.
