The Lead Hazard Evaluation Report (CDPH form 8552) Form Who must file CDPH form 8552?
Any compensated individual who performs an on-site investigation of lead-based paint or lead hazards, including testing and sampling but not the three exceptions in Title 17, California Code of Regulations, Ā§35038, from any public or residential building must submit the Lead Hazard Evaluation Report to the California Department of Public Health (CDPH).
Important - āLead hazard evaluationā means the on-site investigation, for compensation, of lead-based paint or lead hazards for public and residential buildings, but does not include: (a) activities intended to determine adequacy of containment; or (b) air monitoring for lead, as specified in Title 8, California Code of Regulations, section 1532.1, and Title 17, California Code of Regulations, sections 70100 and 70200; or (c) testing components removed from a residential or public building for lead to determine the applicability of hazardous waste requirements specified in Title 22, California Code of Regulations, Division 4.5, Chapters 10, 11, 12, 13, and 18, and California Health and Safety Code, section 25163, subdivision (c). āGeneral Instructions for Filling Out the Forms
The CDPH form 8551 and 8552 will be scanned and entered into a database. Please carefully fill out the form as the applicant shall be held responsible for any mistakes that result in an unreadable form. See the following general guidelines for filling out the form:
- Use standard fonts and avoid stylized writing.
- Use black or blue ball point pen only. Do NOT fill the application form with pencil.
- It is acceptable to utilize a computer program to add text to the designated text boxes on the Lead Hazard Evaluation Report form.
- Write as clearly as possible. Use a pen with the thinnest possible tip.
- Put a check (ā) in the boxes where you must choose one or more options as your answer and leave the other option(s) blank. Do NOT put dots (ā¢), cross marks (įµ”), etc., in the boxes, to choose the appropriate option as your answer.
- Write clearly within the boxes without touching the boundaries. Try to write in the center of the box.
- Avoid folding the Lead Hazard Evaluation Report form more than three times.
- Do NOT smudge the Lead Hazard Evaluation Report form.
- Do NOT write āNAā or āN/Aā or āNOT APPLICABLEā in any boxes on the form. If the field does not apply, leave it blank.
