Addendum #1

This addendum modifies the following Request for Applications (RFA) documents.

Request for Application

1. Part I. E: Updated reference to Teen Talk to Evidence-Informed Program Model (EIPM) on page 12 of the RFA.
   
2. Part I. E: Updated description of Teen Talk Evidence-Informed Program Model classification.
   
3. Part I. E: Replaced inaccurate citation (18) on page 12 of the RFA with an accurate reference to support the Teen Talk curriculum is an evidence-informed program model.
   

4. Part I. K, Table 3: Revised multiple key action dates as follows:

   1. The "Informational webinar" date has been revised from February 14 to February 16, 2022.

Appendix 1 – Population and Community Need Data

1. The link to the CASHNI report was removed on page 1 to correct an error referencing to 2014 data, the CASHNI report for 2018 is coming soon. To find 2018 CASHNI data, please refer to Table 1 in Appendix 1 and page 15, Table 1 in the RFA.
