Infant Formula Information for Health Care Providers 

WIC provides infant formula to WIC participants as a food benefit.

 Standard Contract Formulas

WIC provides contracted formulas as part of a federally mandated cost containment system. In accordance with federal regulations, all WIC formula fed infants may receive any one of the standard contract formulas (PDF) listed below.ā€‹

  Milk-based formulas:
  • Similac Advance
  • Similac Seā€‹ā€‹nsitive
  • Similac Total Comfort
  Soy-based formula:
  • Enfamil ProSobee

 Medical Formulas and Nutritionals

A WIC participant may have a medical condition and need medical formulas or nutritionals that are not part of a WIC formula contract.ā€‹

Health care providers are encouraged to watch this five-minute video or read the letter below to understand the requirements and process to obtain a medical formula or nutritional from WIC.ā€‹ The referral flyer linked below is an additional resource on this topic.

See below for a listing of formulas and nutritionals provided by WIC. If prescribing a medical formula or nutritional that is not on this list, contact Formula@cdph.ca.govā€‹ to inquire about eligibility.
