Antimicrobial Stewardship Program (ASP) Honor Roll
The CDPH HAI ASP Honor Roll application platform, REDCap, is currently down. We have postponed the March 1st deadline indefinitely and will announce a new deadline once the system is back online.
We will be reviewing all applications submitted before the outage using the future deadline. If your facility started an application, all responses have been saved and will be accessible once the system is back online.

The California Department of Public Health (CDPH) Healthcare-Associated Infections (HAI) Program is pleased to recognize top performing antimicrobial stewardship programs (ASP) in California. The goals of the ASP Honor Roll are to promote optimal use of antimicrobials, prevent emergence of antimicrobial resistance andāÆC. difficileāÆinfections, and to showcase California healthcare facility ASP that not only follow national guidelines, but also demonstrate clinically meaningful outcomes from stewardship interventions and engage their local healthcare community.
The ASP Honor Roll updates and expands our programās previous āSpotlight on ASPā project and reflects the substantial progress made with ASP implementation and the increasingly recognized need for ASP across the healthcare continuum. Honor Roll members will need to renew their status every threeāÆyears, providing the member an opportunity to demonstrate continued effectiveness and highlight new achievements.
Benefits of the Honor Roll
- Bronze, Silver, or Gold level ASP designation, with a certificate and an Honor Roll logo that can be displayed on the facility website and other publications
- Recognition on the CDPH HAI Program website and social media
- Demonstration to the public that your facility meets and exceeds national ASP guidelines
- Opportunities to showcase your ASP interventions and provide examples to help other healthcare facilities with their ASP
- Engagement and networking opportunities with the HAI Program Stewardship Teamā and other healthcare facilities to improve antimicrobial stewardship in your local community
Who Should Apply
- We are accepting applications from hospital ASP (e.g., acute care, critical access, long-term acute care); we will expand the programāÆto include skilled nursing facilities (SNF) in the future.
- All hospitals are encouraged to apply.
- Facilities who wish to apply but do not meet Honor Roll criteria may still complete the application and use resources offered through the ASP Honor Roll program including
stewardship consultationsā.
- Individual institutions within a healthcare system need to submit separate applications for each facility.
Instructions and Criteria to Apply

- To receive an application link, please submit an
Application Request Form. The application will then be emailed to you automatically.
- Applications are accepted on a rolling basis with deadlines every six months, March 1st andāÆSeptember 1st.
- Facilities will be notified of their designation status (Bronze, Silver, or Gold) approximately 12 weeks after each deadline. If a facility is awarded Bronze or Silver, that facility can apply for an upgrade at the next 6-month enrollment period. A facility has the option to continue to apply for an upgrade every 6 months (up to one year post full application submission) until they reach Gold.ā
Designation Period
- Facilities enrolled into the Honor Roll will receive a designation
expiring approximately 3 years from the notification date, either May 31st or November 30th.
- For example, applications placed during enrollment period with a September 1, 2025, deadline will be notified and awarded by November 2025. Their designations will expire three years from the award date on November 30th, 2028.
- Applications for upgrades will also have a 3-year expiration date
- Once the desired designation is reached, facilities will need to renew their application every three years. āāāā
Application Review
The CDPH HAI ASP Honor RollāÆapplication āreview processāāÆis a multistage process that includes three rounds of review.
- First, applications are reviewed for completeness and documentation requirements.
- Second, applications are reviewed by the CDPH HAI Infectious Diseases Pharmacist. Applications withāÆPart III: Demonstrations of OutcomesāÆare also reviewed by blinded ASP experts.
- Third, applications are reviewed by the AS team with the CDPH HAI physician and pharmacist antimicrobial stewardship leads to finalize a designation.āÆā
Consultation and Mentorship
The CDPH HAI ASP program offers consultations for facilities interested in strengthening their ASP or improving their Honor Roll applications.
Consultation topics may include technical assistance in applying for the Honor Roll, reviewing AS metrics, developing quality improvement projects or general topic discussion.
