How to Become a California Yellow Fever Vaccine Provider
Uniform stamp certification is required to order and administer yellow fever vaccine. A certified uniform stamp holder has the authority granted by the State of California to administer yellow fever vaccine to the public. The stamp is used for international verification that a traveler has been vaccinated against yellow fever. The stamp is the property of the State of California, Department of Public Health, and is subject to recall or cancellation at the discretion of the Department. Sanofi Pasteur will ship yellow fever vaccine only to a designated facility associated with an active uniform stamp holder. The applicant must be a physician (MD or DO), nurse practitioner, registered nurse, or pharmacist with a current California license.ā

Certification Application Checklist
All applicants are required to submit the following items to become a yellow fever vaccine provider:
- Uniform Stamp application (PDF)
- Proof of completion of the CDC Online Yellow Fever Course by prospective Stamp Holder and Designated Providers.
- ACIP Recommendations & Vaccine Storage Acknowledgement Form (PDF)
- Proof of purchase of the International Certificate of Vaccination or Prophylaxis (ICV) card. (Photocopy of the card will suffice). The ICV card is available for purchase from the Government Printing Office bookstore and from Sanofi Pasteur.
Designating Yellow Fever Vaccine Facilities
Each yellow fever vaccine facility must be under the jurisdiction of an approved stamp holder. Use the
Designation Form (PDF) to list additional facilities where you or a Designated Provider intend to administer the vaccine. CDPH directly notifies Sanofi Pasteur when facilities are approved and have been listed on the CDCāās facility locator site; the vaccine will be shipped only to those sites. (The Designated Providers you name may independently initiate Yellow Fever vaccine orders using your stamp.)
Yellow Fever Stamps
CDPH creates a Purchase Order upon approval of the stamp holder; this PO permits the stamp holder to purchase stamps from the approved vendor. You may request additional stamps for your sites if needed. Each stamp costs about $30.00 including shipping. Stamps no longer include an expiration date and need to be replaced only if worn or lost; however, recertification is still required every three years.
You may request a replacement stamp by submitting the
Uniform Stamp Request for Replacement (PDF). Please return any damaged stamps along with your request.
Vaccine Recipient and Temperature Logs
Certified uniform stamp holders are required to maintain a log of vaccine recipients (including any adverse events or previous YF vaccination) as well as temperature logs for the yellow fever vaccine storage units. Temperatures must be recorded twice a day. You are subject to an audit without notice, and you may be asked to provide these logs for review. Failure to provide this documentation upon request may result in the cancellation of your stamp and ability to order vaccine.
- Vaccination Log, or vaccination records in an electronic information system such as CAIRā. Please include whether the recipient has received yellow fever vaccine in the past and whether there have been any adverse events.

Vaccine Shipping Address Changes
Please submit the
Change of Address Form (PDF) after an address change has occurred at any of your designated sites. Failure to submit this documentation will inhibit your ability to order vaccine. This form must be signed by the uniform stamp holder in order to be processed. The information will be updated accordingly on the
CDC Yellow Fever clinic locator, and with Sanofi Pasteur.
Recertification
Certification expires in three years, on the last day of the month. Stamp holders should take note of the expiration date in the recertification letter, as it is no longer on the stamp itself. Four weeks before the month of renewal, submit a
Recertification Form (PDF) and
Designation Forms (PDF) for all facilities. Please note - Sanofi Pasteur will disallow vaccine ordering by all your designated facilities if your certification expires.
