Changes Questions

1. How do I change (add/remove) a laboratory director?

Currently, online changes are only allowed during renewal.

If you need to make changes to your facility outside of your renewal period, submit the appropriate forms for the type of change by email.

2. How do I change (add/remove) an owner?

Currently, online changes are only allowed during renewal.

If you need to make changes to your facility outside of your renewal period, submit the appropriate forms for the type of change by email.

3. How do I change the laboratory test menu or demographics?

Currently, online changes are only allowed during renewal.

If you need to make changes to your facility outside of your renewal period,
submit by email a LAB 193 (PDF) and a CMS 116 (PDF).​


4. How do I change my CLIA certificate type?

Email us: LFScc@cdph.ca.gov​

5. Can we add a new or additional secondary site if we are not yet renewing?

Yes. There is a charge of $28.00 for each additional site.

  1. Log in to your account.
  2. Look for the specific primary site.
  3. Click the Actions button.
  4. Choose "Add Secondary Sites" and follow the steps.

6. If there were changes during mid-cycle that were not applied, will the renewal date be back dated to the original effective date of the changes?

The renewal expiration date does not change, unless expired for more than 60 days. If changes were not previously processed, you make the changes on your renewal application. The system will ask for the effective dates when necessary. Please click the link for forms that may be required for changes: Submitting Changes​
