TO: The 61 local health jurisdictions (LHJs), including the Directors; Local Health Officers; Deputy Health Officers; and Maternal, Child and Adolescent Health (MCAH) and California Home Visiting Program (CHVP) Directors

CC: California Department of Public Health (CDPH) and state MCAH leadership, and representatives from the County Health Executives Association of California (CHEAC) and the California Conference of Local Health Officers (CCLHO)


​This addendum modifies the following Key ​​Dates in the Release of CDPH/CHVP State General Fund Innovation 2.0 Request for Supplemental Information (RSI)​​ with regards to the Notice of Intent to Award and Launch of Annual Agreement Funding (AFA) Process dates.

The update is reflected in the Key Dates table below and on the State General Fund Innovation 2.0 web page.
