Personal information about individuals is collected through the CDPH website only if an individual provides such information voluntarily through, for example, e-mail, registration forms, or surveys. CDPH will only collect personal information relevant to specific purposes through lawful means, which will be enumerated in the privacy policy or a privacy notice provided at or prior to collection.
