The Centers for Disease Control and Prevention (CDC) created a supplement to the Tuberculosis Elimination and Laboratory Cooperative Agreement (CDC-RFA-PS20- 2001) CFDA number: 93.116; FAIN number: NU52PS910219.

This letter confirms the California Department of Public Health (CDPH) Tuberculosis Control Branch (TBCB) is awarding local assistance funds to support tuberculosis (TB) prevention and control activities serving the recently arrived Ukrainians as part of the Uniting for Ukraine (U4U) program. U4U funds are comprised of federal funds only and must be expended by September 30, 2023.

Managing Your U4U Allotment

U4U supplemental funds, comprised of federal funding only, may be used for: salaries and benefits for staff conducting TB prevention control and elimination activities; food, shelter, incentives and enablers (FSIE) for TB patients and their contacts; TB-related travel; TB-related supplies (excluding equipment); and anti-TB medications. Since funds from the original PS20-2001 (Cooperative Agreement) cannot be used for medications, you will need to keep a separate account of spending for medications purchased with U4U supplemental funds. Unspent U4U funds may be used for TB prevention and control activities that are not part of the U4U program.

Invoicing U4U Expenditures

  • When invoicing for approved expenditures, please:

    • Refer to the FY 20232024 Tuberculosis Control Local Assistance Funds Standards and Procedures Manual, U4U Funding Fact Sheet, and invoice templates available at Resources for Local Health Departments.

    • Bill to: California Department of Public Health, Tuberculosis Control Branch

    • Submit on your organization's letterhead, signed electronically or in blue ink

  • The amount in the “Total" line of Column “A" (Allotment Amount) should reflect the total amount of federal funds awarded to date (“Total U4U Allotment").

  • U4U Allotment invoices are due on:​

  • If an invoice will not be emailed by the quarterly due date, please contact the CDPH TBCB Fiscal Analyst to request an extension.

  • Invoices for the approved budget funding period will not be processed until:

    • Signed copies of the certifications listed below have been received, and

    • The CDPH TBCB has received a signed “Acceptance of Award" form.

Accepting Your U4U Allotment

To acknowledge acceptance of this award and the conditions, please return the “Acceptance of Award" form with an authorized signature (electronic or in blue ink) by October 13, 2023.

 The following forms are required with an authorized signature if not already on file with CDPH for FY 2023​2024:

    • Darfur Contracting Act

    • Special Terms and Conditions

    • Contractor Certification Clauses

    • Certification of Established Electronic Directly Observed Therapy (eDOT) Policies and Procedure
