To support version control, we have added a version number in the document header (e.g., "v2" appears after the document name in the upper right corner of the documents). This practice will continue with future document updates.

1. Appendix 4: Medi-Cal Factor Table (PDF)

   A footer was added to clarify that CDPH/MCAH will utilize the most recent Medi-Cal Factor (MCF) information available during the Agreement Funding Application period, post-award, to determine final Title XIX funding. Appendix 4, Medi-Cal Factor Table, is currently a sample of this information.

   Applicant action: None needed. An updated Appendix 4 (PDF) has been posted.

2. Attachment 3: RFA Program Narrative

   The following issues were addressed in v2 of Attachment 3, RFA Program Narrative:

   • Increased space to type in Agency Name, County, Contact name, and Email Address.

   • Corrected table numbering

   • The instruction for question A9 was updated to include:
     Do not include contracts listed on Attachment 5.

   Note: Most text boxes for narrative responses are intentionally sized to limit response length.

   Applicant action: As of this addendum posting, applicants must download and use Attachment 3: RFA Program Narrative, v2 (PDF).
   

3. Attachment 5: References

   The following issues were addressed in v2 of Attachment 5, References:
   

   • Added organization field​
     

   • The following instruction was updated to read as follows:

     List three (3) contract references where the same or similar scope of services (e.g., case management, home visiting, or other social support services to expectant and parenting persons) were provided to meet the minimum requirements stated in this request for application.

   Applicant action: As of this addendum posting, applicants must download and use Attachment 5: References, v2 (PDF).
   

4. Attachment 6: Government Contracts

   The following issues were addressed in v2 of Attachment 6, Government Contracts:

   • Added organization field

   • The instruction was updated to read as follows (provides details about which contracts to include and adds a minimum and maximum number of contracts):

     List all government contracts through which the applicant has provided service within the last three (3) years that meet the following criteria. Provide two each, if applicable: 1) contracts with the largest budgets; 2) largest budget contracts that were terminated early for any reason; 3) contracts involving case management, home visiting or social services to adolescents, and or expectant or parenting persons. Do not include contracts listed on Attachment 5. The required information may be submitted in alternate format where the information is clearly conveyed, such as a table, and attached as additional pages in Attachment 6. At least five but no more than eight contracts should be listed.

   Applicant action: As of this addendum posting, applicants must download and use Attachment 6: Government Contracts, v2 (PDF).
   

5. Attachment 12: Budget Template

   Tab: FY 23-24: Cells C54-59 and D54-59 for entering and calculating Indirect Costs have been adjusted to properly align with the associated budgeted item. The prior version had an alignment issue causing an incorrect calculation. Other tabs were unaffected.
