āQuestion: The RFA states that we need to submit audited financial statements for the last two years. We currently do not have this. We do have a compilation for 2023 and are in the process of being audited for 2024, but we may not have the audit document ready by the grant application deadline. Would a compilation and other financial documents suffice.
āAnswer: MCAH will accept audited financial statements from 2022 and 2023 if 2024 are not yet available. A compilation or other financial documents will not be accepted in place of audited financial statements.āā

āQuestion: Do you require a COI to demonstrate proof of insurance? If so, is there a specific address and language you would need for us to include?āāāā
Answer: Applicants are required to submit a Certificate of Liability Insurance (COI) as proof of insurance as listed in Part IV.C.8.e. The COI is standardly issued upon request by the insurance company or broker. It usually includes the name of the insured party, the type(s) of coverage, policy numbers, coverage limits, effective and expiration dates, name of the insurer, and certificate holder. Should your insurance provider request an address for the certificate holder you may provide: āā

Question: We are considering partnering with another organization for this grant and would like clarification on the following:āā

Can we submit a joint application, co-writing the grant together?
Answer: Agencies may partner to apply, but one organization must be the primary applicant.

If a joint submission is not allowed and we are listed as a subcontractor, does the primary organization alone need to have five years of experience in QI activities with a focus on maternal and perinatal care collaboration, or can this requirement be met cumulatively between the primary and subcontractor organizations
Answer: Applicant entities and organizations must have a minimum of five (5) years of experience in
promoting QI activities with an emphasis on maternal and perinatal care and care collaboration, including subcontractors.
In the program personnel description, the Applicant describes the proposed staffing and overall personnel plan to meet the SOW Goals. See page 28
