The information you provide to the RHB may be disclosable under the California Public Records Act (PRA). In response to PRA requests, RHB may release the address of record for NMTs. If you wish to provide RHB with an alternate address that will be released to the public release upon request, please submit form CDPH 8391 (PDF).

Application Process Overview

Complete, print, and submit the NMT application via US mail.
Include any fees and/or required documentation listed on the application.
Provide an e-mail address on the application.

Within 30 days of receipt, the RHB will notify you of either of the following:

The application was acceptable and a certificate was issued, or
The application was acceptable and the next steps for an examination, or
The application was not accepted and any next steps.

NMT certificates are issued:

Within 45 days of successful examination completion (when required), or
Within 30 days of receipt of an acceptable applications that does not require an examination.
