Mail-in Payments for Online Application
For this instruction, we will start at the
Application Attestation page. Sign, click
Accept before clicking the
Submit button.
Click the
Shopping Cart button.
Click the
Close button to return to your dashboard. This screen might also serve as your invoice, so you may issue a company check. Take a screenshot of this screen to print.
Your application status should be
Pending Payment. You may now complete and follow the instructions on the Payment Request Sheet (PDF).
