Please mail in the following documents to our office:
- Copy of your CNA/HHA certificate
- Copy Social Security Card
- Copy of California ID/Driver's License
- Social Security Verification obtained from the Social Security Administration Website, verifying the number is assigned to you.
Steps to obtain Social Security Verification
Step 1: Go to the Social Security Administration's website.
- You will need the following:
- Valid email address
- Your social security number
- U.S. mailing address
Step 2: Click āMy Social Security"
Step 3: Click either āCreate an Account" or āSign In"
- If you do not have an account, you need to create an account
- If you have an account, sign in by entering your username and password
Step 4: Read the āTerms of Service" and if you agree, click the box āI agree to the Terms of Service" then click āNext" to proceed to the next screen
Step 5: Page displays your name at the top with the last four digits and date of birth shown to the far right. PRINT THIS PAGE.
