Fees Questions

1. How do I get an invoice?

You can get an invoice by starting a renewal online. Use your computer’s screen shot tool to capture a photo of your Cart showing the Total Fee due. This is temporary, as we are still improving our online application system. Changes to this feature will be updated here.

2. How do I take a screenshot of my screen?

If you’re using Windows, most likely you have a Snipping Tool installed in your computer. Here’s how to use the Snipping Tool.

3. What are the fees for a License?

Laboratory Field Services (LFS) fees are non-refundable and are subject to change with each fiscal year. Refer to the LFS fee schedule for a full list of fees associated clinical laboratory licensing.

5. How are renewal fees calculated?

Registered laboratories (Waived/PPMP testing) have a flat fee.

Licensed laboratories (Certificate of Compliance/or Accreditation) have a fee based on testing volume of the previous year.

6. How are late fees calculated?

Late fees are 25% of the license or registration renewal fee.

7. What are the modes of payment accepted?

For online accounts we accept credit cards, and eCheck.

8. Do you accept checks?

We accept checks with paper applications. Please refer to this user manual for paying by eCheck.

9. Can I get a refund?

Fees are non-refundable. However, a credit is applied towards the next renewal cycle.

Application fees are non-refundable if your application is abandoned, withdrawn, or denied. If a payment was made in error, please contact us at LFScc@cdph.ca.gov with your application and payment details.

10. How do I calculate initial application fees?

Estimate the total anticipated test volume of California specimens to be tested in the first year. Do not include waived test volume. Refer to the Fee Schedule.
